Our Commitment to You

Axis Health Care Services Inc. is committed to providing safe, respectful, professional home health care. Patients and their authorized representatives have the right to participate in decisions about their care, receive information about the services being provided, and raise questions or concerns without fear of retaliation.

Your Rights as a Patient

As a patient of Axis Health Care Services Inc., you have the right to:

• Be treated with courtesy, dignity, respect, and consideration.

• Receive care without discrimination and with respect for your privacy.

• Be informed about the home health services available to you.

• Know the name, role, and professional qualifications of the individuals providing your care.

• Receive information about your condition, treatment, services, and expected course of care in a way you can understand.

• Participate in developing and reviewing your plan of care.

• Request and receive a copy of your current plan of care.

• Be informed about changes in your care or services.

• Ask questions about your care and receive reasonable responses.

• Participate in decisions concerning your treatment and services.

• Refuse a treatment or service and be informed of the possible consequences of that decision.

• Have your health information and clinical records kept confidential as required by law.

• Request access to your medical information as permitted by law.

• Be informed of charges for services and available sources of payment when applicable.

• Receive information about Axis policies that affect your care.

• Be informed about emergency procedures and what to do if your condition changes.

• Have your wishes regarding advance directives respected as permitted by Florida law.

• Receive services without being required to create, change, or waive an advance directive as a condition of receiving care.

• Voice concerns, complaints, or grievances about your care or services without discrimination, interference, coercion, or retaliation.

Florida law specifically provides that a home health patient has the right to be informed of and participate in planning care and to receive a copy of the plan of care upon request.

Your Responsibilities as a Patient

As a patient, you are responsible for:

• Providing accurate and complete information about your health, medications, allergies, medical history, hospitalizations, and current symptoms.

• Telling your nurse or other health care professional about unexpected changes in your condition.

• Asking questions when you do not understand your care, treatment, medications, or instructions.

• Participating in the development of your plan of care.

• Following the agreed upon plan of care or informing your health care professional when you are unable or unwilling to follow it.

• Informing Axis if you refuse a treatment or service.

• Keeping scheduled visits whenever possible and notifying Axis when you need to cancel or reschedule.

• Treating Axis staff and health care professionals with courtesy and respect.

• Providing a safe environment in the home for staff providing care.

• Informing Axis of hospitalizations, emergency room visits, changes in your physician or health care provider, or other significant changes that may affect your care.

• Providing current insurance and payment information when applicable.

• Meeting financial obligations for services for which you are responsible.

Florida’s Patient’s Bill of Rights also recognizes responsibilities such as providing accurate health information, reporting changes, asking questions when instructions are unclear, following the treatment plan, keeping appointments, and meeting financial obligations.

Advance Directives

You have the right to make decisions about your health care, including decisions regarding advance directives.

Axis Health Care Services Inc. does not condition admission or treatment on whether a patient has completed or waived an advance directive.

Adult patients will receive information regarding Florida advance directive rights as part of the admission process when required by law.

Florida home health agency rules require agencies to provide adult patients with written information about Florida advance directive law and the agency’s advance directive policies before services begin.

Complaints and Grievances

AHCA accepts complaints regarding home health agencies and other regulated health care facilities.

Filing a complaint will not affect your right to receive appropriate care from Axis Health Care Services Inc.

Abuse, Neglect, or Exploitation

If you believe a vulnerable adult or child is being abused, neglected, or exploited, immediately contact the appropriate authorities. In an emergency, call 911.

Medical Emergencies

Axis Health Care Services Inc. is not an emergency medical service.

If you are experiencing a medical emergency, call 911 or go to the nearest emergency department.
